Forms: Create, Publish, and Manage Custom Forms

Forms lets you create custom forms — like contact forms, lead capture forms, or inquiry forms — and embed them directly on your website. When a visitor fills one out, the submission is saved in your Admin and can automatically trigger marketing workflows.

Getting Started

To access Forms, navigate to Admin > Online Store > Forms.

From here, you can:

  • Create a new form to collect information from your website visitors
  • View and manage existing forms you've already set up
  • Review submissions from customers who have filled out your forms

Creating and Configuring a Form

When building a form, you have full control over its structure and content:

  • Add fields to capture the information you need (e.g., name, email, phone number, message)
  • Edit fields to update labels, placeholders, or field types at any time
  • Remove fields that are no longer relevant
  • Reorder fields to control how the form appears to your customers

Publishing a Form to Your Website

Once your form is ready, you can publish it to any page on your website. Common use cases include:

  • A "Contact Us" form on a dedicated contact page
  • A lead capture form on your homepage to collect email sign-ups
  • A feedback or inquiry form on the product or landing pages

Viewing Submissions

Every time a customer submits a form on your website, the response is recorded in Admin. To view submissions:

  1. Go to Admin > Online Store > Forms
  2. Select the form you'd like to review
  3. Click the Submissions tab to see all responses

Marketing Integration

Form submissions can be used to power your marketing campaigns. When a customer submits a form, a FormSubmitted event is generated and can be sent to connected marketing platforms like Klaviyo. This allows you to:

  • Trigger automated email flows based on form submissions (e.g., a welcome email after a contact form is submitted)
  • Segment your audience based on the information collected
  • Track engagement from form-driven leads alongside your other marketing data

Tips

  • Use clear, concise field labels so customers know exactly what information to provide
  • Keep forms short — only ask for what you truly need to reduce drop-off
  • Connect your forms to Klaviyo to get the most out of every submission
